Health Clearance Form - The health clearance form is required for all incoming students. Please download and follow the instructions outlined on the form. This form must be completed prior to the start of class and returned to SHS to be medically cleared to start class/clinical.

• Medical and Tan Chingfen Graduate School of Nursing students form

• Morningside Graduate School of Biomedical Sciences MD/PhD form

Respirator Fitness Form - This medical clearance form is to be completed by all first year T.H. Chan School of Medicine and Tan Chingfen Graduate School of Nursing students (Morningside Graduate School of Biomedical Sciences student are excluded). An e-mail will be sent by Student Health to direct you when the form is to be completed. Once medically cleared by Student Health, Environmental Health and Safety will complete the mask fitting process. Please download this form and return to SHS. TST Form - This form must be completed for annual TST testing for ALL Morningside Graduate School of Biomedical Sciences and T.H. Chan School of Medicine students. An e-mail will be sent by Student Health Services directing you to fill out this form during the annual TST clinics (held each spring). If you have had a positive TST in the past, you do not need to attend the TST clinics. However, you must answer the first 2 questions and symptom review section and return to Student Health each year. **NOTE: Morningside Graduate School of Biomedical Sciences students are not required to have annual TST testing. Refusal to Vaccinate Form - Please download this form if you need to defer a vaccination required by Student Health. Refusals will be reviewed with the Student Health Director, Dr. Fournier and your program director to determine if it will affect your course of study. Please return this form to SHS once completed.
